Summary of Position: The RN will be responsible for the professional assessment and management of health conditions/status/indicators of assigned individuals. This is a full-time (40 hours) position based out of our Somers, NY (Westchester County) facility.


  • Certified by NYS Education Department as a Registered Nurse
  • Minimum of 1-2 years’ RN experience
  • Experience working with a developmentally disabled population is preferred
  • A valid Driver’s license.
  • Familiarity with medical software and equipment
  • Ability to build rapport with patients
  • Strong problem solving and critical thinking skills
  • Exceptional organization skills with keen attention to detail.
  • Strong written and verbal communication skills
  • Ability to thrive in a fast-paced environment
  • Ability to travel to various locations


Person Centered Services Specialist

Summary of Position: The Person-Centered Services Specialist is responsible for the development and maintenance of services that are individualized, integrated, and foster independence and productivity within a person centered framework. The person is responsible for the development and implementation of staff action plans for individuals receiving services in conjunction with the service team. The incumbent is responsible for training staff on individuals’ needs and service plans, and is responsible for maintaining all corresponding documentation.


  • Participate in Life Plan meetings for all individuals on caseload and maintain meeting minutes.
  • Create, review and ensure proper implementation of staff action plans for each individual with goals, supports, tasks and safeguards as indicated in the Life Plan, and consistent with the person’s needs.
  • Coordinate and attend special IDT meetings as needed and ensure implementation of any recommendation generated during the meeting
  • Maintain Day Habilitation calendar for all IDT meetings.
  • Complete summaries of individual’s progress in preparation for IDT meetings.
  • Train staff on service plans, observe implementation and meet with staff to monitor individual’s progress.
  • Train staff on completing required documentation and utilizing the electronic health record
  • Perform weekly reviews of data collection to ensure accurate billing of services.
  • Serve as a liaison between staff, family members and care managers as needed.
  • Review Life Plans and attachments for each to ensure accuracy.
  • Provides site-based tours to potential participants and their families.
  • Follows federal, state and local governmental regulatory guidelines pertaining to the safe, healthy and clear work environment, reporting health and safety concerns to the supervisor.
  • Other duties as assigned.


  • Bachelor’s Degree and a minimum of one (1) year experience working with I/DD population OR an Associate’s Degree or equivalent and a minimum of three (3) years’ experience working with I/DD population
  • Sound professional knowledge of programs and treatments available for individuals with developmental disabilities.
  • Good interpersonal skills and the ability to interact well with Individuals, their relatives, advocates, governmental regulatory agencies.
  • Strong writing and organizational skills with proficiency in computer skills.
  • A valid New York State driver’s license.
